    This is a call from a Kaiser facility.  It a roll over #, so that you can't call the caller back.  This person must have been making some follow-up call or contact regarding a visit or if you know someone who works there.  Usually you can not call the number that displays on the phone because they don't want that particular phone to receive calls that are supposed to go through the call center or the exclusive # that belongs to that location.

    So, because Kaiser has an enterprise telephone system any persons who are dialing out from inside the network anywhere in the country may appear to be from that number.

    you  are partially correct the reason why you cannot call the number back its because it's part of a switchboard called a Centrex line those lines are not able to receive calls only outgoing the only inbound calls they are allowed to receive our inter company calls on the same switchboard, usually there's one number within the company that host all the other lines that would be the only number that you could call back and then dial in an extension which would reach that particular number. I hope this does answer your question I know this from my experience of working with the phone company
