    I believe this is Med Care Medical Supplies out of Boca Raton, Fl. trying to sell diabetic supplies. They were caught defrauding  Medicare for over 15 million dollars. Probably changed their name like Liberty Medical( Arriva now) who was also caught defrauding Medicare.
