    I accepted this call after reading comments on this page. Here is my overview.
    The call took about 22 minutes. The questions are very similar to those on patient history forms; however, no questions are required to answer. Questions toward the end pertained to community availability of free exercise programs and walking trails, and the availability of fresh fruits, vegetables, and whole grain foods within one mile of residence.
