Said they were doing a survey about my recent doctors visit and I could call back at this number. The number said it was from Willcox, AZ but the BBB has it listed as out of Ontario Canada.

received 4 calls, didn't answer, they left 4 different messages - this is spam, phishing attempts. Block this number!

The # originated from Ocklawaha, Fl,  352 310 7971.   It said United Healthcare. Left message only telling me to call back the above #  1 888 976 5114. What’s all this about?

Same here-number says it’s Cartersville, Ga-faded out, but assuming it’s UHC since I’ve started getting these types of calls-wanting to do survey of recent doctor visit. I’ve started blocking all calls from UHC. Don’t call me; I’ll call you!

Same here, it's UHC wanting to know about my recent doctor's visit, they don't need to know, my doctor's office sends out their own survey and that's the one I complete.  I have blocked so many numbers from United Health Care, it's unbelievable!

This number has been associated with a variety of scams. Be very careful about calls from this number, included UHC and SS scams.

Neither UHC or SS will ask for your age, address, social security number, bank account number and sign on password. Those questions are BIG Red Flags.
