I just got a call from this number stating that it was Aflac insurance and they were conducting a quality survey. After a few questions, they hung up.

I keep getting calls from the following number, 000-000-0000, how can i identify who this is, the phone company is no help. And it is pissing me off. There is a tone no voice or anything else, is this a fax tone dial what it thinks is another fax machine ?

The number does not exist, can't be called back. It is a spoof number. Anyone can spoof it for any scam at all and that makes finding the caller pretty much impossible. Most call blockers can't block it either.  All you know for certain is that the caller is a scammer.

As to the Fax portion of your question, yes, it is possible this is a lonely fax machine from the late 80's that is lost and trying to find its way home. Look on it with love, it just needs a friend.
